The Clinic complies with the Act on the Protection of Personal Information (the “APPI”), the Guidance for the Appropriate Handling of Personal Information by Medical and Long-term Care Service Providers, and other applicable laws and regulations.
Article 1 (Basic Policy)
The Clinic obtains Patients’ personal information only to the extent necessary to provide medical services and uses or provides it with consent. The Clinic appropriately manages collected personal information to prevent loss, destruction, alteration, or leakage, continually improves its personal-information protection framework, and provides education and training to staff.
Article 2 (Information Collected and Collection Methods)
The Clinic collects the following information:
- (1) Information provided by Patients
Name, date of birth, sex, address, telephone number, email address, information shown on identity documents, questionnaire responses, medical history, medication information, other health information provided by the Patient, and other information entered in forms designated by the Clinic. - (2) Information collected in connection with medical care
Consultation details, prescription details, and other medical records. - (2) To deliver medication and related products and communicate regarding delivery;
- (3) To bill fees and process payments;
- (4) To respond to inquiries and provide necessary information by email or other means;

Article 4 (Handling of Special-care-required Personal Information)
Special-care-required personal information obtained through medical interviews and consultations, including health condition, treatment details, and medication information, is used only to the extent necessary to provide care and improve its quality. It is never used for advertising delivery, advertising measurement, or any other marketing purpose.
Article 5 (Provision to Third Parties)
The Clinic will not provide personal information to third parties without the individual’s consent, except in the following cases:
- (1) When required by law;
- (2) When necessary to protect a person’s life, body, or property and obtaining consent is difficult;

- Providing necessary information to operators of appointment-management, online-consultation, and similar systems.
Article 6 (Advertising)
The Website uses third-party advertising services, including Google Ads and Meta Ads (Facebook and Instagram ads), and uses cookies and other identifiers to display advertisements based on users’ interests. Cookies enable recognition of a user’s browser but do not by themselves identify the individual.
Personalized advertising can be disabled through the following settings:
- Google: the “Ad settings” page in your Google Account
- Meta: “Ad preferences” in Facebook and Instagram
Article 7 (External Transmission of Information for Advertising Measurement)
To measure advertising effectiveness more accurately, the Website may use Google Ads Enhanced Conversions and Meta Ads Conversions API and Advanced Matching.
These features transform contact information entered in appointment and inquiry forms, such as email address, telephone number, and name, using SHA-256 (a one-way hashing method designed to be difficult to reverse), and transmit it to Google LLC and Meta Platforms, Inc. Advertising results are measured by matching this data against account information held by those companies. Transmitted data is limited to the contact information described above; health information such as questionnaire or consultation details is not transmitted. Transmitted information is used solely to measure advertising effectiveness and optimize ad delivery.
Because Google LLC and Meta Platforms, Inc. are located in the United States, use of these features constitutes provision of personal data to third parties in a foreign country under the APPI. For information about the personal-information protection system in the United States, please refer to the Personal Information Protection Commission website’s “Survey of Systems for the Protection of Personal Information in Foreign Countries.” Both companies maintain systems to protect personal data under their respective privacy policies and data-processing terms.
When using forms on the Website, please submit them only after consenting to the handling described above. If you do not consent, you may make an appointment or inquiry by telephone at 0120-49-9629.
Article 8 (Analytics Tools)
The Website uses Google Analytics, an access-analysis tool provided by Google. Google Analytics uses cookies to collect traffic data. Traffic data is collected anonymously and does not identify individuals. Collection can be refused by disabling cookies or using the Google Analytics Opt-out Browser Add-on. For details, please review “How Google uses information from sites or apps that use our services.”
Article 9 (Security Measures)
The Clinic implements necessary and appropriate organizational, personnel, physical, and technical security measures to prevent unauthorized access to, loss, destruction, alteration, leakage, or other compromise of personal information. The Clinic also trains staff on personal-information handling and ensures appropriate practices.
Article 10 (Records of Third-party Provision)
When the Clinic provides personal data to, or receives personal data from, a third party, it creates and retains the required records in accordance with the APPI.
Article 11 (Disclosure, Correction, Suspension of Use, and Similar Requests)
Patients have the right to request disclosure, correction, addition, deletion, suspension of use, or suspension of third-party provision of their retained personal data. To make a request, contact the inquiry desk below. The Clinic may request identification to verify the requester’s identity. After verification, the Clinic will respond without delay in accordance with applicable law, except where the Clinic is not legally required to do so.
Article 12 (Changes to this Privacy Policy)
The Clinic may revise this Policy in response to changes in law or services. Revised terms take effect when posted on the Website. If consent is legally required for a revision, the Clinic will obtain consent by an appropriate method designated by the Clinic.
